Add option to send to extension immediately at the end of IVR

There is currently no way in an IVR just have callers remain on the line to be directed to a queue. There is the time out option where after the IVR repeats three times you can send the call to an extension. We need an option to send the call to an extension if no buttons are pressed after the first time the IVR menus are played.
